If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
Surgery can be performed either in our office surgical suite or in a hospital setting, depending upon the procedure and/or your personal preference. Our offices include a fully accredited and completely staffed operating facility. A board certified anesthesiologist is present whenever sedation is necessary. You will be personally cared for in a safe, secure and comfortable environment while in our office. Complex procedures are performed at one of the hospitals where Dr. Kalvert serves as an attending
surgeon.
All fees will be discussed fully at your first office visit. Charges for consultations and services are due at the time services are rendered. Surgical fees include follow-up visits. As is customary, fees for cosmetic surgery are to
be paid in full in advance of surgery. For your
convenience, credit cards are accepted. We are pleased to assist you in filing your insurance claims where insurance coverage is available. Because insurance coverage can vary, please understand that you are ultimately responsible for settling your account.
